Covid Procedures

• You are required to complete a Mandatory Covid-19 Questionnaire prior to your arrival. If you or anyone in your home are sick please stay home and self isolate. Your appointment will be rescheduled and no cancellation fees will occur.

• Wear a mask and wash or sanitize your hands as soon as you enter the clinic.

• Wait in your car or in the lobby outside the clinic door until we have called you in.
• Appointment times are staggered to promote social distancing.
